;By booking a consultation with me, Diane, you, the client, confirm that you

1. understand that I am a Homeopath practicing from New Zealand and registered with the New Zealand Council of Homeopaths  and therefore abide by the NZCH’s Rules of Practice and Code of Ethics and adhere to legal obligations, rules and guidelines set out in this country; any disputes or claims must initiated and resolved under New Zealand law.

2. understand that I am not a doctor and have no competence to diagnose medical conditions;

3. understand that homeopathy can be used alongside conventional medicines but you must work with your prescribing healthcare professional if you wish to stop or reduce these;

4. consent to me to holding records and personal information/disclosures that include any information you send by electronic or other means & disclosures you make during appointments; I will keep these secure for ten years as per New Zealand law;

5. have considered orthodox medical alternatives and have made a free and informed decision in deciding to use consultation services and homeopathic remedies provided by me;

6. understand that I retain the right and responsibility to refuse consultation in the event that the clients situation is beyond my level of expertise or scope of practice and in order to secure safety in practice for the client as well as myself;

7. acknowledge that you are not representing an agency (private, governmental or otherwise) attempting to gather information without so stating [e.g. you are not a reporter posing as a patient];

8. understand the charges are to be paid on receipt of Invoice unless specific prior arrangements have been made;

9. understand that you will be charged for any appointment that is missed or cancelled with less than 12 hours notice.
